Position Summary:
The Social Worker will serve as a vital member of the Behavioral Health Team at the Old North State Medical Society (ONSMS). This role is responsible for providing culturally responsive, trauma-informed, and person-centered behavioral health services, with an emphasis on health equity and community wellness. The Social Worker will conduct assessments, coordinate care, provide individual and group interventions, and assist in connecting clients to needed social, emotional, and behavioral supports.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments and develop individualized care plans.
- Provide short-term counseling, crisis intervention, and psychoeducation to clients and families.
- Collaborate with primary care providers, psychiatrists, and other behavioral health professionals to deliver integrated care.
- Facilitate client access to community resources including housing, food, transportation, and social services.
- Advocate for client needs within medical, legal, and community systems.
- Maintain accurate and timely clinical documentation in compliance with HIPAA and organizational standards.
- Participate in case conferences, interdisciplinary team meetings, and program planning efforts.
- Engage in outreach and education initiatives to promote behavioral health awareness in underserved communities.
- Support evaluation and reporting activities related to grant-funded programs and outcomes.
- Uphold the mission of ONSMS in reducing health disparities and improving behavioral health access among historically marginalized populations.
Required Qualifications:
- Master’s degree in Social Work (MSW) from a CSWE-accredited program.
- Active North Carolina LCSW or LCSWA license.
- Minimum 3-5 years’ experience in behavioral health, community mental health, or integrated care settings.
- Knowledge of evidence-based interventions for mood disorders, trauma, substance use, and chronic stress.
- Demonstrated commitment to equity, inclusion, and cultural humility in service delivery.
- Strong communication, documentation, and organizational skills.
- Ability to work both independently and collaboratively in interdisciplinary environments.
Preferred Qualifications:
- Bilingual (English/Spanish or other language) skills strongly preferred.
- Experience working with BIPOC, LGBTQ+, and rural communities.
- Familiarity with trauma-informed care and motivational interviewing.
- Proficiency in electronic health record (EHR) systems.
Work Environment & Conditions:
- Hybrid schedule with in-person clinic, community-based, and remote work components.
- Occasional evenings or weekends for outreach or community events.
- Must have reliable transportation for travel to community sites when needed.
