Behavioral Health Team Lead (Licensed Nurse – RN/NP or Physician Assistant)

Position Summary
The Behavioral Health Team Lead is a licensed clinical professional Registered Nurse or Physician Assistant. This is the person who supervises and coordinates the day-to-day operations of ONSMS’s behavioral health team. This role ensures the delivery of high-quality behavioral and mental health care through leadership, clinical referrals, mitigation of barriers to health care access, patient-centered practice in community-based and office settings.
Key Responsibilities
Clinical Leadership & Coordination
- Conduct and/or supervise mental health pre-assessments, conduct other simple wellness vitality checks.
- Make provider health referrals in accordance with outlined training.
- Interact with community members to deliver health education sessions, disperse materials, promote physical and mental health well-being
- Lead regular Behavioral Health Team (BHT) meetings to discuss events, partnerships, delivery processes, planning, etc.
- Reports to Program Coordinator
Team Supervision & Professional Development
- Supervise and mentor behavioral health staff, including social workers, medical assistants, and case managers.
- Provide orientation, clinical guidance, and ongoing feedback/coaching.
- Model and reinforce trauma-informed care, cultural humility, and wellness-oriented practices throughout the team.
Program & Quality Management
- Oversee intake processes, care coordination, and documentation in line with HIPAA and regulatory standards.
- Ensure compliance with state/federal regulations and ONSMS quality assurance protocols.
- Analyze program data and outcomes for reports and continuous improvement efforts.
Patient Care & Outreach
- Support crisis intervention, including emergency protocols and de-escalation strategies.
- Conduct or delegate routine participant check ups for referral follow through
- Participate in community events and partnerships to expand behavioral health access in underserved communities.
Administrative Contributions
- Serve as liaison with internal and external stakeholders, including primary care, behavioral health providers, and community partners.
- Contribute to grant-funded initiatives through planning, implementation oversight, and data reporting.
- Assist with resource allocation, including staffing, supplies, and clinical materials.
Required Qualifications
- Licensure:
- RN with active NP certification in Behavioral Health/Psychiatric Specialty OR
- Physician Assistant with state license and documented supervised behavioral health experience.
- Minimum of 3-5 years of clinical experience in behavioral health settings, including supervisory or lead responsibilities.
- Demonstrated knowledge of mental health disorders, evidence-based practices, medication management, and documentation standards.
Preferred Qualifications
- Experience in outpatient, community, or integrated primary care behavioral health.
- Strong leadership, communication, and conflict resolution skills.
- Bilingual (Spanish/English) or cultural competency with diverse populations.
- Familiarity with trauma-informed, recovery-focused, and integrated care models.
- Experience capturing and analyzing health outcome metrics and quality measures.
Work Environment & Expectations
- Hybrid: primarily in-office with community visits and outreach.
- Flexibility in schedule, including potential on-call duties for patient crises.
- Travel required within the service area; reliable vehicle essential.
